And in 1890 London, a young woman named Elizabeth narrowly escapes becoming the next victim of Jack the Ripper, only to be drawn into the Witch's sinister web of secrets.

In the present day, Detective Katrina Jenson investigates a series of ritualistic murders, the latest victim displaying a gruesome "blood eagle" method of torture. Partnered with the mysterious Detective Jacobs, Katrina must unravel the supernatural connections between these crimes and the Witch's ancient evil. Meanwhile, Sam is drawn into the Witch's machinations when he is tasked with locating a missing woman named Annabelle Scavenger, a lead that puts him on a collision course with the dark forces at work.

As Sam tracks Annabelle to a remote forest, he witnesses a disturbing ritual and rescues her from the clutches of the Witch's cult. However, their escape is cut short when they are ambushed at a gas station, with Sam brutally beaten and Annabelle taken captive once more. Awakening in a nightmarish otherworldly realm, Sam is confronted with visions of his family's torment, a pig-like creature sexually assaulting his wife Sally while his son serves them.

Imprisoned in the Witch's lair, Annabelle watches in horror as the Witch sexually assaults the unconscious Sam. When Annabelle attempts to flee, the Witch's demonic servants brutally break her ankle, trapping her in a cage. The Witch then reveals her tragic backstory, explaining how she was ostracized by her village, leading her to make a dark pact that granted her supernatural powers to raise an army for a malevolent dark lord.

As the Witch's machinations continue to unfold, Detective Katrina and her new partner Jacobs delve deeper into the investigation, uncovering the disturbing historical connections that link the ritualistic murders to the Witch's ancient evil.
